Getting started is easy:
- Schedule an appointment – call our office at (817) 467-1300 and our friendly staff will be happy to help you find a convenient appointment time.
- Complete new patient forms – during your first visit, we’ll ask you to complete a few initial forms. To save time, you may download and complete them before your appointment here.
- Bring the following to your first visit:
- Your child’s insurance card
- A valid photo ID for the parent or guardian
- A list of your child’s current medications, including doses
